Notice of privacy practices
This notice describes how health information about you may be used and disclosed, and how you can get access to that information. Please review it carefully. A signed copy is provided at intake.
Our commitment
Your health information is confidential. It is used only to provide your care, to obtain payment, and to conduct the limited administrative work required to run the practice. Records are stored in a secure, HIPAA-compliant electronic record system.
Uses that require your written authorization
Releasing your evaluation report or clinical records to a physician, school, attorney, employer, or family member requires your written authorization. You may revoke that authorization in writing at any time, except where information has already been released in reliance on it. Psychotherapy notes and most marketing uses always require authorization.
Uses permitted without authorization
The law permits or requires disclosure without your authorization in limited circumstances: a serious and imminent threat to your safety or the safety of another person; suspected abuse or neglect of a child, elder, or vulnerable adult; a valid court order; certain public health and health-oversight requirements; and coordination with a professional consultant, who is bound by the same confidentiality.
Your rights
You have the right to inspect and receive a copy of your record, to request an amendment, to request restrictions on certain uses, to ask for confidential communication by a particular method or address, to receive an accounting of disclosures, to be notified of a breach affecting your information, and to receive a paper copy of this notice.
Telehealth
Telepsychology is provided under PSYPACT authority (APIT Provider #20538) using an encrypted, HIPAA-compliant platform. Sessions are not recorded.
